Take Heart Program (Esperance)
The Take Heart Program
Based in Esperance this program supports clients and their partners with the provision of a rehabilitation program and participation in other initiatives promoting adoption of healthier lifestyles.
The Take Heart Cardiac Rehabilitation Program targets those recovering from a cardiac condition or those with significant risk of developing coronary artery disease. This program provide a combined program of exercise and education to assist clients in becoming physically active to achieve health benefits, and encourage lifestyle modification for improved health.
The GEGPN employs a Registered Nurse (Sue Mitchell) and an Exercise Professional (Cris Bradley). A joint collaboration occurs with Esperance Community Health to provide prevention strategies related to healthy living.
On completion of the initial program, a further more strenuous exercise program is offered to those clients that which to continue with exercise under supervision.
